We present two patients who developed tracheobronchomalacia (TBM) following allogeneic haematopoietic stem cell transplantation (HSCT) and were initially treated for post-transplant bronchiolitis obliterans syndrome (BOS) but did not experience symptom improvement. However, after treatment with continuous positive airway pressure, their symptom subsided. |

An example of ventilatory limitation during cardiopulmonary exercise testing in a patient with COPD

A 64-year-old gentleman with obesity and moderate COPD attended for further evaluation of ongoing dyspnoea despite treatment with dual long acting bronchodilators. A cardiopulmonary exercise test (CPET) was performed, which demonstrated reduced peak work and oxygen consumption with evidence of dynamic hyperinflation, abnormal gas exchange and ventilatory limitation despite cardiac reserve. The CPET clarified the physiological process underpinning the patient's dyspnoea and limiting the patient's activities. This, in turn, helped the clinician tailor the patient's management plan. |

An 81 year old male with Child-Pugh A cirrhosis and metastatic hepatocellular carcinoma (HCC) treated with 3-weekly atezolizumab and bevacizumab developed a pulmonary sarcoid-like reaction (SLR) after 5 months. Atezolizumab was identified as the likely culprit. He was treated with prednisolone, resulting in improvement, and was successfully rechallenged with both atezolizumab and bevacizumab. |

We herein report a rare case of endobronchial mucosa-associated lymphoid tissue (MALT) lymphoma (EML) that was diagnosed through a chest computed tomography (CT) during an annual health checkup which was successfully treated with radiotherapy. To the best of our knowledge, only 48 cases of EML have been reported in the literature and are summarized here. |

A woman in her mid-50s was admitted to our hospital with airway stenosis secondary to mediastinal lymph node enlargement and the AERO stent was placed under rigid bronchoscopy. Metastatic lesions were found to have an EGFR mutation. The tumour achieved a partial response to osimertinib, and the patient underwent stent removal. |

We report a rare case of pneumonia and bacteremia caused by Bacillus subtilis subsp. natto in an immunocompetent patient. B. subtilis subsp. natto is a gram-positive spore-forming bacteria and a subspecies of B. subtilis. Natto is a Japanese traditional food made by fermenting soybeans with B. subtilis subsp. natto which can produce nattokinase. |

This clinical case demonstrates that in specific contexts, EBUS-cryobiopsy represents an excellent diagnostic tool. A young man was brought to our attention after already undergoing a non-diagnostic trans thoracic needle aspiration (TTNA) procedure for lung consolidations. In our department, he underwent an initial EBUS-TBNA procedure with inconclusive rapid on-site evaluation (ROSE), leading to the decision to perform an EBUS-guided cryobiopsy, which yielded a diagnosis of granulomatosis with polyangiitis without complications. |

Delayed diagnosis of Swyer-James-MacLeod syndrome

Swyer-James-MacLeod Syndrome is a rare obliterative lung disease typically caused by childhood infection resulting in arrested pulmonary development. Imaging findings include unilateral hyperlucency on chest x-ray, and hyperlucency, hypovascularity and expiratory gas trapping on computed tomography. Recognition of abnormal imaging can lead to earlier diagnosis and institution of appropriate management. |
Target sign of the lung

We present the first report of a case of a radiological target sign that was potentially associated with drug-induced lung injury. A target sign of the lung consists of a ring-like opacity and a central nodular opacity and has been limited so far to reports associated with SARS-CoV-2 infection or organizing pneumonia. |
Glandular papilloma in lung parenchyma

We report a rare case of glandular papilloma in lung parenchyma. The glandular type of solitary respiratory papilloma (Glandular papilloma, GP) is the rarest type of these rare epithelial tumours. Most GP occurs in the proximal airways and is only rarely found in the lung parenchyma. |
